- Immigration Physicals
- Occ Med &Worker's Comp Services (such as Drug Testing, First Aid, pre-employment physicals, DOT physicals)
- Primary Care Services
Services performed by outside facilities (such as specialist visits and lab work done outside the clinic, outside imaging, hospitalization, ambulance transportation).
- Membership Fee per Member is $45.00/month.
- Each Member must pay a utilization fee of $15.00 per visit at the start of each urgent care visit.
- To join, each Member must pay first & last month’s fees plus enrollment fee (plus applicable tax).
- Membership Fees must be paid and deducted by automatic transfer from Visa, MasterCard, or American Express account.
- Membership may be canceled with written notice in accordance with the terms of the Urgent Access Membership Agreement.
- Membership may not be combined with any other discount offers.
